Guidelines for Submitting Campus Data

The deadline for submitting this survey is August 1, 2002

At the request of the presidents, the task of responding to this survey has been assigned to the campus president's designee to the Information Technology Advisory Committee. On most campuses, the ITAC designee is responsible for the technology resources and support services covered by this survey. The ITAC designee is expected to undertake whatever consultation is necessary to obtain the requested information from campus associates.

Following are suggestions aimed at minimizing frustration and mistakes in the initial use of this web survey interface.

Data submission as the FINAL step
The active web page is constructed as a single page. The database will accept only one submission per campus. No changes can be made after the form has been submitted. If you close the web page for any reason, the form will be cleared and all data you have entered will be lost. We recommend, therefore, that data be entered into the interactive web form from a completed hardcopy of the questionnaire. In addition to simplifying the survey submission process, the completed questionnaire provides an archival reference for future surveys.

Access to the survey questionnaire
One of the main reasons for moving the survey process from paper to the web is to facilitate the extensive consultation required to provide the requested information. The survey questionnaire, including the explanatory links, can be viewed and downloaded by anyone. If you wish to have it for internal use, a blank copy of the FileMaker Pro database that we are using to store the data can be sent to you on request.

Authorization to submit data
Campus technology survey coordinators will receive by email a password for submitting the survey data. Our assumption is that any data submitted for your campus has the approval of the campus technology survey coordinator. The first submission with the campus name is accepted as the only campus response. Please make sure, therefore, that you (or your designee) retain control over submission on behalf of your campus.

Password placement
The request to enter the password appears only AFTER you click on the submit button.

Numerical values
In reporting dollar amounts ($) and numbers round off values to the nearest whole number. Report FTE values and percentages to the nearest tenth. Enter percentage as decimal number; e.g., for 35% enter .35, for 100% enter 1.0

Accessing clarifications and returning to the survey
Clarifications and interpretations of terms used in the survey can be accessed by clicking on the bold brown text fragments. Use the Back button on your browser to return to questionnaire.

Error messages
Error messages will appear for the following reasons:

1. one or more of the required fields (indicated by an asterisk) was omitted;
2. data have already been received from your campus;
3. the password is incorrect.
